NYS Changes to required physical examinations and dental certificates
Parents/guardians – please take note: New York State has changed the grades for required physical examinations and dental certificates. Beginning with the 2018-19 school year, all students must submit documentation of a physical exam and a dental exam when they first enter the district (all PreK students, some Kindergartners, any kids in older grades who move into the district) and in grades 1, 3, 5, 7, 9 and 11. If you have a child entering any of these grades in the fall, please contact your healthcare provider and dentist to schedule an appointment. If your student has had a physical or dental exam on or after Sept. 1, 2017, that exam may be used for the 2018-19 school year.
