1095 -C Information:
Many of you have had good questions about the 1095-C you received. The following are a few frequently asked questions, we hope this helps everyone’s understanding.
Who should receive a Form 1095-C?
If you are under the age of 65, and enrolled in health insurance through your former employer at any time during this tax year, you will receive a Form 1095-C.
What do I do with the Form 1095-C?
Keep this form with the materials you give to your tax preparer. If you prepare your own taxes, you may need to refer to this information as you prepare your return.
Why does Part III of my Form 1095-C have check marks and blanks?
Part III pertains to Employer provided self-insured (NY44 Health Benefits Trust) or fully insured (MVP PPO) coverage.
January through June the district was self-insured coverage through NY44 Health Benefits Trust Plan, this is the reason for the checked boxes during those months.
Effective, July 1, 2018 the district went to a fully insured coverage through MVP PPO, which is the reason the boxes are blank from July through December.
