Student Registration

Welcome to the Elmira City School District! We look forward to working with you and your children during the school year. In an effort to make the registration process easier for you, our registration process allows you to register all of your students* at any one school in our district.

To Register a student, we will request:

  • A copy of each student's Birth Certificate
  • Three (3) Proofs of Residency - Lease, rent receipt, Utility bill, DSS Statement, or other acceptable proof
  • F001 Student Registration Form
  • F002 Household Information Form
  • A copy of each student's updated immunization and health records with the completed F003 Student Health Information and History Form and Dental Health Certificate for each student
  • NYS Education Department requires: F004 Home Language Questionnaire Form for each student
  • Federal Law requires: F005 Housing Questionnaire Form
  • Parent/Guardian Picture Identification
  • Name and contact information for 2 local emergency contacts
  • Custody Papers indicating residential custody (if applicable)
  • F006 Records Release Form for each student
  • F007 Parent Portal Registration Form
  • F008 Household Income Eligibility Form for the National School Lunch Program

During the summer, schools are open for registration usually from 8:00am to 3:00pm. During the school year, schools are open for registration during the regular school day.

*Pre-K students must still be registered at District Administration due to program limitations. Please call (607) 735-3055 for more information.

Registration Tips to Make the Process Easier

  1. Before leaving your former district, please get or have copies of the following documents:
    - Immunization Records
    - Copy of the most recent IEP or 504 Plan (if applicable)
    - Custody Papers indicating residential custody (if applicable)
    - School Records/Transcript or Final/Most Recent Report Card or Withdrawal Grades

    Bringing this information with you when you register your student(s) can help us get them placed into school in a more timely fashion.
  2. You should register your student(s) as soon as possible. If you move in the summer, please do not wait until September to register them. We get overcrowded at that time and waiting may cause your student(s) to start later than everyone else.
  3. You can download the registration forms and documents below and complete them prior to going to the schools for registration. This will speed up the registration process.

These forms will be needed to complete registration. You can fill them out and print them prior to registration in order to make the process easier. They are in PDF format. You will need Adobe Reader or other PDF software on your computer to complete.
