Per the guidelines set forth by the New York State Department of Health, parents and guardians of hybrid students enrolled in the Elmira City School District will receive a daily home health screen by email each school morning that the student will attend in-person.
The GST BOCES COVID SCREENING will be sent from
[email protected] by 5:30 a.m. Please be sure to check your SPAM/Junk email folders if you don’t see a message from this address in your Inbox.
If you do not receive an automated email, please check your child’s health and temperature before sending him/her to school in the morning and use the screening questionnaire below.
If you answer YES to any of the questions, do not send your student to school.
1. Have you been in close or proximate contact in the past 14 days with anyone who has tested positive through a diagnostic test for COVID-19 or who has had symptoms of COVID-19?

3. Have you experienced any symptoms of COVID-19, including a temperature of greater than 100.0℉, in the past 14 days?

4. Have you travelled internationally or from a state with widespread community transmission of COVID-19 and is restricted per the New York State Travel Advisory in the past 14 days?
